The Clinical Nurse Coordinator is a key member of the Transitional Care Unit team at SNCH, responsible for providing clinical leadership and ensuring the delivery of high-quality patient care. This role requires a strong nursing background, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities- Provide clinical leadership and guidance to nursing staff, ensuring the delivery of evidence-based care.
- Develop and implement policies and procedures to improve patient outcomes and enhance the overall quality of care.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct regular audits and quality improvement initiatives to identify areas for improvement and implement changes.
- Develop and maintain relationships with patients, families, and healthcare providers to ensure a positive and supportive care environment.
- Current and valid New York State registration authorizing practice as a Registered Professional Nurse (RN).
- Two years of current SNF/Sub-acute Rehab nursing experience, with a strong background in clinical leadership and staff development.
- Excellent interpersonal and communication skills, with the ability to work effectively in a fast-paced environment.
- Public relations, team-building, problem-solving, decision-making, conflict management, analytical, and critical thinking skills.
- Computer skills and proficiency in electronic medical records.
This role requires frequent, prolonged standing and walking, as well as the ability to lift, position, and transfer patients and equipment. Personal Protective Equipment (PPE) will be worn as necessary.
