The Rehabilitation Specialist is responsible for assessing and treating residents in accordance with physician directives, utilizing therapeutic techniques and assistive devices aimed at optimizing functional abilities across all aspects of a resident's life.

This role involves collaboration as part of an interdisciplinary team, offering support and insights in the realm of Occupational Therapy.


Key Responsibilities:

  • Analyze available resident information and assess its influence on the evaluation and treatment process.
  • Conduct and document a comprehensive functional assessment of the resident, employing standardized methodologies. Assess home/vocational and equipment requirements.
  • Formulate and execute a personalized treatment plan tailored to resident needs, incorporating achievable functional objectives.
  • Adhere to evaluation, treatment, and documentation protocols while consistently maintaining accurate and timely departmental records.
  • Document progress notes in the resident record following each treatment session.

Qualifications:

  • Bachelor's or Master's degree in Occupational Therapy.
  • Current licensure/certification in the relevant state.
  • Preferred experience, training, or supervision in a setting catering to a similar demographic as the resident population served by this facility.
