This part-time Inpatient Palliative Care Social Worker position is an excellent opportunity to make a meaningful impact in the lives of patients and their families.
Key Responsibilities- Assess patients and their families to identify social, emotional, economic, and environmental challenges and develop interventions to address these issues.
- Collaborate with the healthcare team to promote, monitor, and evaluate compliance with patient treatment plans.
- Assist with discharge planning and coordinate referrals to community resources, including transportation, housing, healthcare, and social services.
- Administer and interpret tests and measures of psychosocial functioning to diagnose mental, emotional, behavioral, addictive, and developmental disorders and disabilities.
- Supervise and train new staff, lower-level social workers, students, or interns.
- Master's degree in Social Work.
- State licensure as a Licensed Clinical Social Worker (LCSW).
- Commitment to obtaining Palliative Care certification within 5 years.
