Clinical Psychosocial Specialist

As a Clinical Psychosocial Specialist at Kaiser Permanente, you will provide comprehensive psychosocial services to adult and child members in a primary or specialty care setting. Your expertise will include evaluation of psychosocial assets and deficits, short-term counseling, education, advocacy, advanced care planning, and resource brokering to enable and maintain independence. You will work under the supervision of a Licensed Clinical Independent Social Worker, Licensed Marriage Family Therapist, or Licensed Professional Clinical Counselor to provide mental health and addiction assessment, triage, diagnosis, treatment, and crisis intervention services.
You will collaborate with treating primary or specialty care providers and psychiatric, addiction medicine, and allied health professional team members to plan and direct each individual patient's treatment program. Your role will involve conferring and consulting with supervisors regarding members' progress and treatment, providing support to members' families, and utilizing resources of public and private agencies and community organizations to meet members' treatment needs.
You will participate in departmental program development, implementation, and evaluation, and report safety concerns to mandated reporting agencies. Your responsibilities will include developing, implementing, coordinating, and evaluating plans of care and/or clinical treatment programs for members with acute or chronic mental or addiction illness.
You will instruct and counsel patients and their families regarding compliance with plans of care, administer specialized therapeutic interventions as appropriate, and complete all documentation of members' treatment and progress in accordance with Kaiser Permanente, state, and NCQA regulations.
Essential Responsibilities:
Confers and consults with supervisor regarding members' progress and treatment
Provides support to members' families, including explanation of treatment, instructions in how to support treatment, and interventions to increase acceptance of and adherence to treatment
Utilizes resources of public and private agencies and community organizations to meet members' treatment needs
Participates in departmental program development, implementation, and evaluation
