FeldCare Connects is a pioneering app-based network dedicated to providing exceptional home healthcare services. We are seeking a self-motivated Physical Therapy Assistant to join our team and contribute to the well-being of patients of all ages.

About the Role

As a Physical Therapy Assistant in Home Health, you will be responsible for delivering patient care services that enhance mobility, reduce pain, restore function, and prevent disability. You will work closely with our Physical Therapists to ensure that treatment plans are followed, and interventions are implemented to improve patient outcomes.

Key Responsibilities
  • Provide services defined by state laws governing the practice of a Physical Therapy Assistant, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Help patients use therapeutic, creative, and self-care activities to improve functioning and safety.
  • Observe, record, and report patient responses to treatment and changes to patient conditions.
  • Instruct patients, families, caregivers, and the healthcare team in areas of therapy where they can participate in assisting patients.
Qualifications

To be successful in this role, you will need:

  • Ability and enthusiasm in working with all ages.
  • Physical Therapy Assistant license and registration by the state.
  • Completion of a Physical Therapy Assistant curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association.
  • Bilingual is a plus.
