Job Description:

The Healthcare Liaison Professional will be responsible for working with patients to assess gaps in care and developing plans to address them. This includes providing brief interventions at point of care, advocating for patient services with community, social service, and medical providers, and participating in morning huddles to anticipate patients' clinical, social, and behavioral health needs.

Responsibilities:

  • Assess patients' needs and develop plans to address gaps in care.
  • Provide education and support to patients on chronic disease management and prevention.
  • Collaborate with the care team to improve Patient-Centered Medical Home processes.
  • Document patient information accurately and efficiently.

Requirements:

  • A minimum of 1 year of experience in a healthcare setting, preferably in a community health organization.
  • Familiarity with electronic health records and Microsoft Office software.
  • Strong communication and interpersonal skills, with the ability to establish a therapeutic relationship with patients.
