Liberty Healthcare & Rehabilitation Services is seeking a skilled MDS Coordinator to join our team.

Job Summary:

We are looking for a detail-oriented and organized individual to maintain and follow a schedule of due dates for all MDS. The successful candidate will coordinate the completion of the MDS by all disciplines, review data for accuracy, and meet with staff as needed to ensure accuracy.

Responsibilities:

  • Maintain and follow a schedule of due dates for all MDS.
  • Coordinate the completion of the MDS by all disciplines.
  • Review MDS data for accuracy and meet with staff as needed to ensure accuracy.
  • Complete the MDS and input into the computer.
  • Verify that assessments have been transmitted and approved by the State in a timely manner.
  • Correct any rejected records and prepare them for re-submission.
  • Complete CAAs according to state guidelines.
  • Formulate Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
  • Schedule and conduct RCP meetings on a regular and timely basis.
  • Communicate the RCP to appropriate staff members and notify supervisor if noncompliance by staff is noted.
  • Work with all in-house and ancillary departments to ensure understanding and compliance with the RCP.

Requirements:

  • Registered Nurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
  • Ability to read, know, and follow personnel, department, and facility policies and procedures and adhere to local state and federal requirements.
  • Experience with MDS-RAP and Care Planning functions.
  • Prefer experience with RUG-IV.
  • Prefer experience with MDS 3.0.
