Key Responsibilities:
  • Develop and implement Personalized Service Plans (PSPs) in collaboration with residents and their families
  • Provide hands-on care, including assistance with daily living activities, such as bathing, dressing, and grooming
  • Assist with medication management, including administering medications and monitoring side effects
  • Engage residents in activities and programs that promote socialization, mental stimulation, and overall well-being
  • Monitor and report any changes in resident condition, including falls, injuries, or illnesses
  • Collaborate with other care team members to ensure seamless care delivery and resident satisfaction
Requirements:
  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred
  • Previous experience working with elderly or disabled individuals, preferably in an assisted living or dementia care setting
  • Ability to work flexible hours, including weekends, evenings, and holidays
  • Knowledge of dementia care principles and practices, including person-centered care and behavior management
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and care team members
