Nurse Care Specialist

**Key Responsibilities:**
* Provide skilled nursing care to patients in their homes
* Conduct thorough assessments and develop individualized care plans
* Administer medications and treatments as prescribed by physicians
* Monitor patient progress and report any changes or concerns to the Director of Clinical Nursing
* Collaborate with other healthcare professionals to ensure comprehensive care

* Valid LPN license
* Annual physical within one year
