Position Overview:

Access North Center for Independent Living of NE MN is seeking a Part-Time Case Manager in the Bemidji region. Key Responsibilities:

  • Manage daily operations and growth of Home and Community-Based Services (HCBS) including 245D and PCA services.
  • Ensure quality service delivery and compliance with relevant regulations.
  • Provide training, oversight, and performance evaluations for 245D and PCA staff.
  • Assess the effectiveness of services provided to consumers.

Role Duties:

The Case Manager will be responsible for:

  • Overseeing operational and administrative tasks to meet the objectives of Access North's in-home support services.
  • Engaging in networking, marketing, and outreach efforts with local counties, service providers, agencies, and community organizations.
  • Supervising and training staff to ensure compliance with Direct Support Professional (DSP) standards.
  • Maintaining accurate documentation of services rendered and time spent with consumers.
  • Building and maintaining relationships with county social workers and waiver specialists.
  • Collaborating with HR to identify and fulfill DSP staffing needs.
  • Ensuring comprehensive training for DSPs related to 245D and PCA compliance and person-centered practices.
