Job Summary

We are seeking a skilled Registered Nurse to join our team as a Post-Acute Care Specialist. In this role, you will provide high-quality patient care in post-acute facilities, collaborating with medical staff and residents to develop care plans and track outcomes. Your expertise and leadership skills will be essential in ensuring the accurate documentation of medical histories and interventions, as well as supervising nurses and support staff.

Responsibilities
  • Provide high-quality patient care in post-acute facilities
  • Collaborate with medical staff and residents to develop care plans
  • Track outcomes and review options with physicians and other nursing staff
  • Administer and supervise appropriate treatments and medications to residents
  • Ensure medical equipment is properly set up and monitored
  • Inform residents and family members about best care options
  • Draw blood, urine, and other fluids for lab work
  • Ensure staff compliance with facility policies and nursing standards
  • Verify accurate documentation of medical histories and interventions
  • Supervise nurses and support staff, providing training as needed
  • Assist with incident investigations
Requirements
  • Current Pennsylvania state license to practice as a Registered Nurse (RN)
  • Graduation from approved nursing program
  • At least 4 months of experience in post-acute or in-patient hospital care
  • Excellent organizational and leadership skills
