About the Position: We are looking for a detail-oriented Care Coordinator to join our team. As a Care Coordinator, you will be responsible for managing patient care and coordinating treatment plans. Your duties will include handling administrative tasks, serving as a liaison between patients and medical staff, and streamlining communication to ensure patient satisfaction.

Your Key Responsibilities Will Include:

  • Managing patient care and coordinating treatment plans
  • Handling administrative tasks and duties for the physician/provider
  • Serving as a liaison or coordinator for the patients medical care
  • Streamlining all patient-physician communications to ensure patient satisfaction
  • Providing medical literature and clinical preparation instructions to patients
  • Assisting patients with questions and/or concerns regarding procedures
  • Scheduling all procedures to be performed by the physician
  • Reviewing the physicians schedule for maximum scheduling efficiency
  • Scheduling all diagnostic tests, procedures and follow-up appointments
  • Obtaining all authorizations for procedures and tests
  • Calling patient to confirm procedures a week in advance
  • Scheduling follow-up appointments including recalls
  • Checking-out patients at the end of their visit and providing next step instructions
  • Requesting medical records from doctors and hospitals
  • Returning patient calls promptly and professionally
  • Calling-in new prescriptions and refills and obtaining authorization if necessary
  • Obtaining lab results including stat requests
  • Completing tasks from Electronic Medical Record
  • Reviewing open orders every three days and working accordingly
  • Contacting patients with test results
  • Sending history and physical forms to outpatient facility

What You Need:

  • High school diploma or GED equivalent
  • Certified Medical Assistant (AAMA) preferred
  • 2+ years experience as medical assistant required
  • Medical terminology knowledge
  • Bilingual (English/Spanish) required
