Job Description

Brockton Area Multi-Servi is seeking a highly skilled Nurse Director to oversee the healthcare needs of individuals living in assigned residential programs.

Key Responsibilities:

  • Clinical Supervision: Provide clinical supervision to Assistant Nurse Directors, conduct formal written supervisions, and performance appraisals.
  • Nursing Staff Management: Assist with interviewing and hiring nurses, participate in hiring events, and meet weekly with assigned Talent Acquisition Specialists to review vacancies.

Case Management:

  • Screening Referrals: Assist with screening referrals, visit nursing homes, community residences, and hospitals as needed to obtain referral information, and coordinate team transition meetings with funders, guardians, and BAMSI team.
  • Hospital Admissions/Discharges: Coordinate hospital admissions and discharges, follow up with individuals served after ER visits or inpatient hospitalizations, and coordinate with hospital and nursing home case management teams to plan for discharge.
  • Home Health Agency Coordination: Work with partnering home health agencies to secure assessments, therapies, and equipment for individuals served.
  • Reporting and Documentation: Review and finalize HCSIS reports post-ER/hospitalization, complete written reports, assessments, and other documentation as required.

Training and Development:

  • Person-Centered Trainings: Recommend person-centered trainings post-hospitalization, review person-centered trainings prior to staff training, and ensure content is aligned with ISP requirements.
  • BAMSI MAP Training: Participate in BAMSI MAP training program, become a MAP trainer within 6 months of hire, and complete courses including MAP, MAP recert, and MAP Overview for Managers.
  • Ehana Trainings: Participate in Ehana trainings as needed.
  • Medical Trainings: Review medical trainings, make recommendations based on agency need, and ensure staff are trained on SIC and nurse competency.

Quality Compliance:

  • MAP Compliance: Provide clinical and administrative oversight to programs regarding DPH Medication Administration Program (MAP) procedures, protocols, and compliance, and coordinate quarterly MAP reviews using the MAP Technical Tool.
  • Licensing Audits: Conduct semi-annual licensing audits of medical records at programs in the service area using the BAMSI Medical Licensing Review Tool.
  • Investigations and Inquiries: Participate in investigations and inquiries, providing content expertise and explanation of agency processes and practices.

Additional Responsibilities:

  • Cluster Meetings and Trainings: Facilitate cluster meetings and on-going program-specific medical trainings on diagnosis, protocols, and other healthcare and wellness topics.
  • Team Environment: Promote a positive team environment, ensure Positive Behavior Supports (PBS) are implemented, and maintain professional boundaries.
  • Program Oversight: Participate in the planning and oversight of programs in the absence of an Assistant Nurse Director.
  • On-Call Responsibilities: Provide 24-hour on-call support, ensure nursing coverage, and provide medical support and access to resources.
  • Staffing Coordination: Collaborate with staffing coordinators to ensure the centralized system for coordinating nursing coverage is implemented effectively.
  • Senior Meetings: Attend and participate in senior nursing and director meetings.

Qualifications:

  • Education/Credentials: Registered Nurse in the Commonwealth of Massachusetts, valid driver's license in state of residence.
  • Experience: Minimum of five (5) years nursing experience or three (3) years supervisory experience required, knowledge of theories, methods, procedures, and practices of nursing care.
  • Preferred Qualifications: Two years experience working with individuals with disabilities.

Knowledge/Skills/Abilities:

  • Computer Literacy: Basic computer literacy, including the ability to record data, use electronic time and attendance software, receive and send electronic mail, and work with the agency-approved electronic health care record.
  • Critical Thinking and Communication: Effective critical thinking, communication, and writing skills.
  • Organizational and Interpersonal Skills: Strong organizational, time management, interpersonal skills, and the ability to prioritize responsibilities and work within a team environment.
  • Physical Demands: Ability to push, pull, and lift at least 25 lbs.
