About the Role

We are seeking a skilled RN Care Manager to join our team at the Family Medicine Clinic at Rio Rancho High Resort. As a key member of our primary care team, you will play a vital role in facilitating collaborative care between patients, families, physicians, and other healthcare professionals.

Responsibilities
  • Proactively manage patient care along a continuum, ensuring seamless transitions and optimal outcomes.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and timely interventions.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
Requirements
  • Registered Nurse (RN) licensure in the State of New Mexico or Compact.
  • 1-3 years of ambulatory experience.
  • Associate Degree in Nursing.
