Role Summary: The Case Manager IV delivers clinically-focused case management to enhance the effectiveness and efficiency of patient care. This role encompasses utilization management, care coordination, and transition planning. The Case Manager IV is accountable for a designated case load, ensuring that patient needs are met, length of stay is managed, and resources are utilized efficiently. Collaborating with the interdisciplinary team, the Case Manager IV aids the physician in facilitating patient care, aiming to improve patient outcomes and satisfaction while managing care costs and providing timely information to payers.

Qualifications

Additional Information:
*Registered Nurse with a Master of Science in Nursing (MSN) and five years of experience in utilization review or case management, or a Registered Nurse with a Bachelor of Science in Nursing (BSN) and ten years of experience in utilization review or case management is required.
*Current Registered Nurse licensure in the State of New Mexico is required.
*Two years of clinical nursing experience in a relevant area is necessary.
*National Case Management certification is preferred.

Education Requirements:
Essential: Bachelor’s Degree
Specialization: Nursing

Credentials Required:
Essential: Registered Nurse - NM

BLS Certification is required at the time of hire.
