- Patient Monitoring: Continuously monitor patients' conditions, identifying potential issues and taking proactive steps to address them.
- Support and Education: Connect patients with support groups and services, educating them on disease management and prevention strategies.
- Health Promotion: Promote optimal wellness and healthy lifestyles among patients, empowering them to take control of their health.
- Acute Illness Management: Participate in the management of acute illnesses, working collaboratively with healthcare teams to ensure effective care.
- Chronic Disease Management: Assist patients in managing the effects of chronic diseases and disabilities, providing emotional support and guidance.
- End-of-Life Care: Provide compassionate support to patients and their families during end-of-life care, ensuring dignity and respect.
