Program Coordinator

Position Summary:
A Program Specialist is responsible for overseeing the effective operation of the program in alignment with Chapter 6400 and Title 55 PA Code CH 51. This role involves reporting to the Executive Director and includes incident reporting in HCSIS and EIM, as well as staff training. The Program Specialist will also be tasked with the continuous development and execution of provider services, requiring a solid understanding of HCSIS and EIM systems.
Key Responsibilities:
The specific duties include:
- Preparing and submitting monthly and quarterly reports.
- Monitoring, interpreting, and analyzing participant goals, and drafting monthly progress reports based on these evaluations.
- Participating in ISP meetings and contributing to the formulation of the ISP and its annual updates.
- Reviewing the ISP, conducting annual updates, and making necessary revisions.
- Reporting any discrepancies in content to the Service Coordinator and relevant team members.
- Implementing the ISP as outlined.
- Supervising, monitoring, and assessing the services provided to individuals.
- Reviewing, signing, and dating monthly documentation for participants.
- Engaging in the progress towards defined outcomes.
- Completing mandatory training sessions as required.
- Ensuring compliance with Chapter 6400 and Title 55 PA Code CH 51.
- Coordinating and conducting assessments.
- Providing required assessments under the relevant chapter and distributing them to each team member.
- Communicating any changes related to individual needs to the Service Coordinator.
- Reviewing the ISP with the individual and documenting the discussion.
- Providing documentation of the ISP review to the Service Coordinator and team members as necessary.
- Informing team members about the option to decline the ISP review.
- Recommending revisions to services or outcomes in the ISP.
- Coordinating services provided to individuals.
- Documenting all incidents in EIM and ensuring compliance, with closure within 30 days.
- Verifying that all participants' ISPs are current and properly organized.
Qualifications:
To qualify for this position, candidates must meet one of the following criteria:
- Master's Degree with 1 year of experience working directly with individuals with Intellectual Disabilities.
- Bachelor's Degree with 2 years of experience working directly with individuals with Intellectual Disabilities.
- Associate's Degree or 60 credit hours with 4 years of experience working directly with individuals with Intellectual Disabilities.
Job Type: Full-time
Salary: $40,000 - $50,000 per year
