Rheumatology Specialist

We are seeking a skilled Rheumatologist to join our team at AvitaCare Atlanta. As a Rheumatologist, you will provide patient and clinician consultations, creating diagnostic and treatment care plans. You will also function as part of our integrated care team, dedicated to providing whole-person care in an outpatient setting.
Key Responsibilities:- Provide care for people living with chronic rheumatologic conditions, including inflammatory and degenerative arthritides, autoimmune diseases, osteoporosis, and related disorders.
- Conduct outpatient care consultations, creating diagnostic and treatment care plans.
- Provide clinician consultation services, chiefly to internal clinicians but also to collaborating practices and external clinical entities.
- Develop an intensive and comprehensive model of outpatient care for patients with complex chronic diseases, collaborating with clinic leaders, other specialists, and care team members.
- Work in a team environment, collaborating with social work, care management, integrated behavioral health, pharmacy, nutrition, health education, and other care team members.
- Implement plans of care in collaboration with the patient, their family, and other team members.
- Monitor patient conditions to minimize emergency department visits, admissions to the hospital, and readmissions.
- Assist with educating patients regarding their chronic diseases and healthy choices.
- Provide on-call coverage in rotation with other providers.
- Supervise and collaborate with assigned advanced practice providers (APPs, including NPs and PAs) as needed.
- Perform other related duties as assigned.
We are looking for a Medical Doctor with a strong background in Rheumatology. You should have a Medical degree from an accredited university, be Licensed Physician in the state of Georgia, and Board certified following specialized training. Experience in population health management and working with psychosocially and medically complex patients is preferred. You should also have knowledge or experience with the NCQA PCMH model and be able to work effectively as part of a multidisciplinary team.
