Rheumatology Specialist

As a Rheumatologist at AvitaCare Atlanta, you will provide patient and clinician consultations, creating diagnostic and treatment care plans. You will work in a team environment, collaborating with social work, care management, integrated behavioral health, pharmacy, nutrition, health education, and other specialists to develop an intensive and comprehensive model of outpatient care for patients with complex chronic diseases.
You will conduct outpatient care consultations, provide clinician consultation services, and implement plans of care in collaboration with the patient, their family, and other team members. You will also monitor patient conditions to minimize emergency department visits, admissions to the hospital, and readmissions.
About You:
- Medical degree from an accredited university
- Licensed Physician in the state of Georgia
- Board certified following specialized training
- American Heart Association BLS certification
- Experience in population health management preferred
- Experience in working with psychosocially and medically complex patients strongly preferred
- Knowledge or experience with NCQA PCMH model preferred
