Nursing Care Specialist

Job Summary:
We are seeking a skilled Licensed Practical Nurse to join our team at Penn Medicine Lancaster General Health. As a key member of our healthcare team, you will provide high-quality nursing care to patients in a variety of settings.
Key Responsibilities:
- Provide clinical advice by phone or electronic correspondence based on clinical judgments and/or triage protocols.
- Consult with providers as necessary to ensure the best possible patient outcomes.
- Provide patient education on preventive care, chronic disease management, acute diagnoses, medications, and other areas pertinent to patient outcomes.
- Administer injections and medications as prescribed by the provider and in accordance with department protocol, regulatory requirements, and standards of practice.
- Prepare patients for visits by using appropriate rooming standards tools.
- Perform Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
- Collect blood or other laboratory specimens and prepare them for further testing.
- Authorize prescription refills as instructed by the provider.
- Assist with test tracking and monitoring overdue results.
- Remove sutures and change dressings on wounds as necessary.
- Assure exam rooms are adequately supplied and cleaned after each patient visit.
- Maintain and clean medical instruments and equipment after each procedure.
- Assist providers and patients in reviewing and revising care plans and assess patient progress.
- Follow up with patients between visits as necessary.
- Assist with proactive outreach for patients requiring preventive and chronic disease services.
- Perform care coordination activities among specialists and transitions of care to obtain all necessary patient information.
- Assist in coordination of referrals, education, and community resources.
- Actively participate in performance improvement initiatives to improve overall patient experience and quality of care.
- Participate in daily huddles with the provider.
- Serve as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors, and colleagues.
- Communicate with all patients, visitors, and colleagues in a professional manner while keeping customer service a top priority.
- Handle all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures.
Requirements:
- Completion of a LPN or a RN educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing.
- AHA CPR/BLS certification required within 30 days of hire.
- This role requires occasional regional travel. Ability to travel to multiple sites is required.
Preferred Qualifications:
- One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office.
- Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience.
