POSITION SUMMARY

Heluna Health is dedicated to enhancing community health through innovative programs and services. The Facility Improvement Assistant Analyst (FIAA) role is pivotal in supporting our mission by ensuring the quality and safety of residential care facilities serving vulnerable populations, including individuals experiencing homelessness. This position operates under a flexible work model, allowing for both remote and in-office work as necessary.

The FIAA will work closely with the Director of the Enriched Residential Care Program (ERC), overseeing the operations of facilities that provide essential housing and support for individuals requiring assistance with daily living activities. The primary responsibility of the FIAA is to manage a designated portfolio of Residential Care Facilities for the Elderly (RCFEs) and Adult Residential Facilities (ARFs) within the region, ensuring high standards of service delivery and client care.

KEY RESPONSIBILITIES
  • Conduct regular on-site evaluations of assigned ERC facilities to assess safety and service quality.
  • Utilize assessment tools to grade facilities and identify areas for improvement.
  • Develop and implement improvement plans addressing identified concerns, including necessary training and resources.
  • Provide on-site technical support in collaboration with the Referral and Placement Team and client wellness teams.
  • Identify and evaluate new facilities while monitoring existing ones for compliance and quality.
  • Maintain communication with relevant stakeholders regarding facility status and client safety.
  • Ensure the master facility list is current and accurate within the designated management system.
  • Facilitate quarterly learning communities for facility operators to share best practices and address challenges.
  • Coordinate internal team meetings to discuss facility performance and training needs.
  • Oversee contracts and agreements related to ERC facilities, ensuring compliance and effective reporting.
  • Identify training themes for facility administrators based on observed trends and challenges.
  • Participate in ongoing technical assistance and learning community meetings.
QUALIFICATIONS

Applicants should possess a minimum of three years of experience in a role focused on analyzing organizational issues and recommending solutions, or two years of highly responsible administrative experience, or one year of relevant experience in health care or social services. Experience in programs that provide housing and support to at-risk populations is highly desirable.

CERTIFICATIONS/LICENSES
  • Valid California Class C Driver License or alternative transportation method for job-related duties.
  • Successful completion of the Live Scan and Health Clearance processes with the County of Los Angeles.
SKILLS AND COMPETENCIES
  • Experience in homeless health care and clinical service delivery within RCFEs and ARFs.
  • Knowledge of complex care management for high-need MediCal beneficiaries.
  • Ability to lead performance improvement initiatives and effectively communicate with diverse teams.
  • Expertise in managing clients with substance use disorders and serious mental health issues.
  • Familiarity with trauma-informed care and harm reduction strategies.
WORK ENVIRONMENT
