Role Overview:
As a Complex Care Physician, you will lead a dedicated team of advanced practice providers, social workers, and medical assistants. Your primary focus will be on enhancing the quality of life for high-risk patients and their caregivers, enabling them to age comfortably at home while minimizing hospitalizations.

Key Responsibilities:

  • Provide direct patient care through in-clinic, home, and virtual visits.
  • Conduct hospital rounds to oversee care and ensure optimal discharge plans.
  • Perform emergency department evaluations for appropriate triaging.
  • Lead palliative care services and coordinate with other high-risk teams.
  • Manage patient and primary care provider feedback effectively.

Qualifications:
To be successful in this role, you should possess:

  • An active and current medical license.
  • Board certification in Internal or Family Medicine.
  • A minimum of 3 years of clinical experience with complex patients.
  • Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
  • Familiarity with Electronic Medical Record (EMR) systems.
