We are seeking a skilled Wound Care Specialist to join our team at AlignMed Partners. As a key member of our healthcare team, you will provide high-quality care to our patients, focusing on wound management and treatment.
Responsibilities- Evaluate patient health status, diagnose, and interpret lab results
- Develop and implement patient treatment plans in collaboration with interdisciplinary care teams
- Conduct weekly wound rounds and document patient progress
- Assess, manage, and treat wounds, identifying underlying causes and developing effective treatment plans
- Address quality of life concerns related to wounds and develop curative, maintenance, or palliative wound care goals
- Master's Degree in Nursing from an accredited program
- Graduate of an accredited Nurse Practitioner Program with a concentration in Older Adult or Family Nurse Practitioner
- Wound Certification (CWS, WOCN, or equivalent) or actively pursuing certification
- Minimum of two years' experience in wound care, working with adult or geriatric populations
