About the Role

We are seeking a highly skilled Cardiology Clinical Pharmacy Specialist to join our Ambulatory Care team at Community Health Network. As a key member of our interprofessional team, you will play a vital role in providing exceptional patient care and advancing the level of care to achieve the best possible outcomes.

Key Responsibilities
  • Manage hypertension, dyslipidemia, cardiovascular risk reduction, antiarrhythmic monitoring, tobacco cessation, DOACs and VKA anticoagulation service, as well as transitional care management for cardiology discharge patients both in-person and virtually.
  • Provide staff education, guideline/policy development and implementation, and participation in performance improvement initiatives and practice advancement.
  • Mentor and teach pharmacy students and pharmacy residents, and serve as a clinical resource for the healthcare team.
  • Advance the level of patient care to achieve the best possible outcomes.
Requirements
  • Strong communication skills and a collaborative approach to patient care.
  • Ability to work closely with physicians and healthcare teams, while providing direct patient care.
  • Ability to work closely with patients to overcome medication adherence issues.
  • Ability to serve as a drug information resource for other providers and clinic staff, answering medication therapy questions.
  • Ability to monitor for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests.
  • Ability to engage in population health projects to impact the lives of patients we serve.
  • Clear understanding of quality improvement principles.
  • Ability to precept pharmacy students and residents.
  • Graduate of an accredited college of pharmacy, Bachelor or PharmD; Doctorate degree.
  • Indiana Pharmacist license.
  • Successful completion of a PGY1 Pharmacy Residency and PGY2 residency with Cardiology or Ambulatory Care focus is strongly encouraged or a minimum of three (3) years working under a drug therapy agreement in a clinical setting.
  • Board certification as a Board Certified Pharmacotherapy Specialist, Board Certified Ambulatory Care Pharmacist, Board Certified Cardiology Pharmacist, or Certified Anticoagulation Care Provider is encouraged and required within 2 years of accepting the position.
