Clinical Healthcare Specialist

We offer a rewarding opportunity for a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. As a Clinical Healthcare Specialist, you will play a vital role in improving the quality of care for older adults and at-risk individuals.
About the Role:
- You will conduct comprehensive health assessments in patients' homes, reviewing their medical history, medication adherence, and social environment.
- Based on your findings, you will provide personalized health education and recommendations to improve patients' overall well-being.
- You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
Responsibilities:
- Conduct home visits, skilled facility visits, and telehealth visits to assess patients' health status.
- Review patients' medical history, medication lists, and social determinants of health.
- Develop and implement personalized care plans to improve patients' health outcomes.
- Collaborate with healthcare teams to ensure timely and effective care coordination.
Requirements:
- Masters degree in Nursing or Physician Assistant Studies.
- Current RN and NP or PA licensure in state of practice with prescription authority.
- Board certification by a national certifying organization.
- Minimum 1 year experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with electronic medical records.
