As a key member of the CHRISTUS Health team, the successful candidate will be responsible for delivering high-quality, patient-centered care to individuals within our health system. This role requires a strong understanding of healthcare principles and the ability to coordinate care with medical directors to ensure comprehensive treatment plans are in place.
Key Responsibilities:
- Conduct thorough patient assessments, including medical histories and physical exams, to inform treatment plans.
- Order and interpret laboratory studies to diagnose and manage chronic diseases.
- Develop and implement individualized care plans in collaboration with medical directors and other healthcare professionals.
- Provide education and support to patients and their families to promote healthy outcomes.
- Collaborate with healthcare teams to ensure seamless care transitions and effective communication.
Requirements:
- Current licensure through the Texas Board of Nursing or licensed Physician Assistant by the Texas State Medical Board.
- Valid DEA license.
- BLS certification obtained from the American Heart Association.
Work Environment:
This is a full-time position that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced healthcare environment.
