Job Title: LPN - Home Health - Days

INTEGRIS Health is seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team in Oklahoma City. As a LPN, you will provide exceptional care to patients in their homes, working collaboratively with our interdisciplinary team to deliver high-quality patient outcomes.

Responsibilities:
  • Perform patient assignments as coordinated with the Care Manager, Clinical Supervisor, and scheduler under the direction of the specialized skills as per agency policies/procedures
  • Ensure implementation of interventions for improved quality of patient care and outcomes per established plan of care
  • Report changes in patient status to the provider and initiate any new orders from the provider as directed
  • Instruct the patient/caregiver in signs and symptoms of complications and home management related to disease processes as outlined in the established plan of care
Requirements:
  • Oklahoma LPN license
  • 1 year of LPN experience
  • Home Care experience preferred
  • Current BLS certification
  • Ability to communicate effectively in English
