About the Role:

As a Hospice Aide with Traditions Health, you will play a critical role in delivering high-quality patient care in the comfort of their own homes. Your primary function will be to provide personal healthcare and related services to patients, including assistance with daily living activities, medication management, and mobility support.

Key responsibilities include:
  • Providing direct patient care under the direction of a registered nurse.
  • Assisting patients with safe transfers and ambulation, adhering to Agency policy.
  • Reporting changes and documenting pertinent information and care rendered to patients.
  • Promoting positive communication and respect for patients' privacy and property.

Requirements:

To succeed in this role, you must possess the following qualifications:
  • Education: High School Diploma preferred.
  • Licensure: Current driver's license required.
  • Experience: At least one year of experience as a Home Health Aide or Nursing Assistant.
  • Skills: Ability to read, write, and verbally report clinical information; proof of current CPR certification.
