Job Summary:

The Registered Nurse Case Manager will be responsible for planning, organizing, and directing hospice care services. This includes conducting comprehensive assessments, developing and implementing individualized care plans, and coordinating with community health resources to ensure seamless care delivery.

Key Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans that address physical, emotional, and spiritual needs
  • Coordinate with community health resources to ensure timely and effective care delivery
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
  • Graduate of an accredited school of nursing
  • Current licensure in the state
  • 1-2 years of recent acute care experience in an institutional setting, and 1-2 years of recent experience in home care preferred
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
