Job Summary:

We are seeking a highly motivated and compassionate Case Manager to join our team at Service Coordination Inc. (SCI). As a Case Manager, you will play a vital role in providing support and advocacy to individuals with developmental disabilities, ensuring they receive the necessary resources and services to achieve their goals.

Key Responsibilities:
  • Establish and maintain positive relationships with individuals, families, and community stakeholders
  • Conduct comprehensive assessments and develop Person-Centered Plans to meet the unique needs of each individual
  • Coordinate and facilitate access to community resources, services, and support
  • Advocate on behalf of individuals to ensure their rights and interests are protected
  • Monitor and evaluate the effectiveness of services and make recommendations for improvement
Requirements:
  • Bachelor's degree in a human services field or related field
  • 2+ years of experience working with individuals with developmental disabilities
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
