RN Home Health Specialist

This RN role will serve the southwest territory of our West Michigan home care service area, traveling to patient homes between Grand Rapids and Gun Lake.
Key Responsibilities- Assess, develop, and maintain individualized patient plans of care with patients.
- Communicate effectively with staff, residents, and families.
- Perform medication administration passes according to regulatory guidelines.
- Inform MDS coordinators of significant resident/patient changes needed for MDS completion.
- Communicate with other healthcare team members to ensure continuity of care and coordination of services.
- Act as patient/family advocates.
- Evaluate patient's and significant other's educational needs and develop and implement teaching strategies or make referrals.
- Observe and report patient conditions, needs, and outcomes of intervention to physicians and other team members.
- Identify problems and emergency situations and initiate immediate life-saving measures in the absence of a physician.
- Implement plans of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures.
- Regularly evaluate patient progress toward established goals and recommend revisions to the plan as necessary.
- Plan for termination of service or discharge.
- Assume leadership roles and delegate appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.
- Participate in ongoing process improvement activities through the application of clinical paths, QA processes, and other related activities.
- Associate's Degree in Nursing.
- 1 year of relevant experience in acute care, emergency, or rehab facility experience for all Home Care business lines.
- LIC-Registered Nurse (RN) - State of Michigan.
- LIC-Driver's License - State of Michigan.
- CRT-Basic Life Support (BLS) - AHA or ARC.
This role requires frequent standing, walking, and driving. The ability to lift up to 50 pounds and push/pull up to 25 pounds is necessary. The role also requires occasional sitting, forward bending, trunk rotation, and stair climbing.
