About the Role

As a Home Health RN Case Manager, you will provide high-quality, personalized care to patients in the comfort of their own homes. You will work closely with a team of healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans that address patients' unique needs
  • Provide education and support to patients and their families on disease management, medication, and home safety
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Document patient progress and communicate with healthcare teams as needed

Requirements
