  • Schedule visits with assigned patients based on care requirements.

  • Foster strong working relationships with the hospice team during the case management process.
  • Proactively engage with patients to advocate for their end-of-life preferences.

Qualifications:
  • Demonstrated experience in hospice care.
  • Strong communication abilities.

  • Robust clinical knowledge to manage complex patient requirements.
  • Empathetic and compassionate demeanor.
  • Current LVN licensure.
  • CPR certification.
  • Valid driver's license and dependable transportation.
