Position Summary:
We are in search of a dedicated and empathetic Advanced Practice Provider to deliver comprehensive healthcare services to patients in long-term care facilities. This role is vital in ensuring that our residents receive the highest standard of care while collaborating with a team of committed healthcare professionals.
Key Responsibilities:
- Deliver holistic healthcare services and manage patient care effectively.
- Collaborate with a multidisciplinary team of healthcare providers.
- Evaluate, diagnose, and treat both physical and mental health conditions of residents.
- Perform detailed patient evaluations, including reviewing medical histories and conducting physical assessments.
- Order and analyze diagnostic tests to inform treatment decisions.
- Formulate precise diagnoses and develop individualized treatment plans.
- Prescribe medications and oversee treatment protocols.
- Provide guidance and support to residents and their families.
- Work closely with the interdisciplinary team to ensure seamless care coordination.
- Manage chronic health issues and track patient progress.
- Adjust care plans as necessary to meet evolving patient needs.
- Uphold a commitment to person-centered care.
- Foster a safe and supportive environment for elderly residents.
Qualifications:
- Valid Nurse Practitioner or Physician Assistant License.
- DEA License required.
- Minimum of 6 months experience in long-term care settings.
Benefits Offered:

Salary Range: $60,000 - $90,000.00
