Job Summary

The Clinical Pharmacist at Akicita Federal, LLC provides expert consultation services to ensure the optimal use of medications. This role involves reviewing patient medication profiles, counseling patients, and providing in-service training to clinical staff. The pharmacist also participates in interdisciplinary team meetings, drug utilization reviews, and inventory control. Key responsibilities include:

Key Responsibilities
  • Provide consultation services to patients and clinical staff.
  • Review patient medication profiles to monitor and optimize therapy.
  • Ensure appropriate use of medications, including dosage, interactions, duplications, and allergy information.
  • Compound medications as required.
  • Provide clinical staff with medication information and therapy guidelines.
  • Offer in-service training to staff on new medications and appropriate use.
  • Participate in the Sole Provider program and maintain the opioid registry.
  • Provide clinical direction to pharmacy technicians and supervise inventory control of pharmaceuticals.
  • Maintain records and security of all controlled substances in compliance with regulations.
  • Consult with specialists to ensure optimal pharmaceutical care for patients.
  • Report Adverse Drug Reactions (ADR) in line with Joint Commission (JC) standards.
  • Facilitate medical staff drug utilization reviews (DURs) and monitor pain medications in collaboration with the interdisciplinary team.
  • Participate in patient medication monitoring programs and risk management meetings.
  • Assist in identifying educational needs for staff and develop appropriate strategies.
  • Participate in interdisciplinary team meetings to review patient cases.
  • Ensure compliance with Joint Commission standards and maintain confidentiality.
  • Provide in-scope administrative duties, including record-keeping and participation in performance improvement activities.
Requirements
  • Doctoral degree in Pharmacy from an ACPE-accredited institution.
  • Completed a Pharmacy Residency or equivalent experience.
  • Proficiency with Microsoft Office and Electronic Medical Record (EMR) systems.
  • Valid, unrestricted pharmacy license from any U.S. state or territory, with a preference for a Maryland state license.
  • Current National Provider Identifier (NPI) and Drug Enforcement Administration (DEA) registration.
  • CPR certification.
Preferred Qualifications
  • At least one year of experience working as part of an interdisciplinary team.
  • Minimum of one year of experience as a practicing pharmacist.
  • Experience teaching residents, patients, or healthcare students within the last three years.
