Patient Care Specialist for Palliative Services

We are seeking a skilled Patient Care Specialist to join our team in Boston, Massachusetts. The successful candidate will provide high-quality palliative care services to patients and their families, working closely with our multidisciplinary team.
Job SummaryThis is a full-time position, requiring 40 hours of work per week. As a Patient Care Specialist, you will be responsible for assessing patients' psychosocial needs, developing comprehensive care plans, and providing individualized support to patients and their families.

Key Responsibilities- Conduct thorough psychosocial assessments of patients to identify their needs and develop personalized care plans.
- Provide individualized support to patients and their families, addressing their emotional, social, and practical needs.
- Collaborate with our multidisciplinary team to ensure seamless care coordination and effective communication.
- Develop and maintain relationships with community resources to ensure access to essential services for patients and families.
- Bachelor's degree in Social Work or related field required; Master's degree preferred.
- Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) in Massachusetts required.
- Previous experience in palliative care or a related field highly desirable.
- Competitive salary: $83,000 - $100,000 per year, based on experience.
