We are seeking a compassionate and skilled Medical Social Worker to join our team at Community Loving Care Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing medical social services to our patients and their families.

Key Responsibilities
  • Assist patients and their families in understanding the social and emotional factors related to their health status, and provide coping mechanisms to manage these challenges.
  • Collaborate with the healthcare team to develop a comprehensive plan of care that addresses the patient's physical, emotional, and social needs.
  • Provide education and support to patients and their families on community resources available to assist with their care.
  • Function as a bereavement coordinator and supervise the provision of bereavement services to meet the unique needs of each patient and their family.
Requirements
  • Bachelor's degree in social work from a school accredited by the Council on Social Work Education or a related field.
  • One year of experience in a healthcare setting, preferably in social work or a related field.
  • Current CPR certification and a valid driver's license.
