We are seeking a highly motivated and compassionate Case Manager to join our Rapid Rehousing Services Team at PATH (People Assisting the Homeless). As a Case Manager, you will play a critical role in assisting clients in breaking the cycle of homelessness by providing individualized support and connecting them to essential resources.
Key Responsibilities- Conduct Intake and Needs Assessment: Meet with clients to understand their unique needs and develop a personalized plan to address their housing goals, income, and other personal objectives.
- Provide Referrals and Linkages: Connect clients with local resources, services, and support systems to help them achieve their goals.
- Document and Maintain Client Files: Ensure accurate and up-to-date records of client interactions, progress, and outcomes.
- Maintain Confidentiality: Uphold the highest standards of confidentiality and adhere to HIPAA guidelines.
- Education and Experience: A minimum of 2 years of experience in healthcare, case management, homeless services, or a related field.
- Analytical and Communication Skills: Possess strong analytical and communication skills, with the ability to work effectively with diverse populations.
- Driving Requirements: A valid driver's license, reliable transportation, and proof of insurance are mandatory.
