Job Summary

As a Home Health Aide with Cantex Continuing Care Network, you will provide personal healthcare and related services to clients in their place of residence. Your primary function is to assist in providing a safe and clean environment, work cooperatively with patients and families, and share observations and problems with your supervisor.

Responsibilities
  • Provide direct patient care under the direction of a registered nurse and according to the Aide Plan of Care.
  • Assist patients with safe transfers and ambulation, as per agency policy.
  • Report changes and document pertinent information and care rendered to patients to ensure continuity of care.
  • Practice accepted infection control principles and provide a clean, safe, and comfortable environment.
  • Promote positive, respectful communication to patients, families, and other employees.
  • Contribute to the management and efficient operation of the agency, demonstrating effective time management skills.
  • Perform treatments and procedures according to agency policy.
  • Participate in weekly case conferences and interdisciplinary communication to ensure patients' needs and expectations for services provided are understood.
Requirements
  • High School Diploma preferred.
  • Current driver's license.
  • At least one year experience as a Home Health Aide or Nursing Assistant in a hospital, nursing home, or home health agency, or completion of a Certified Nursing Assistant or Certified Home Health Aide Program.
  • Good communication skills to relate professionally and positively to patients, caregivers, and clinical teams.
  • Ability to read, write, and speak the English language.
  • Demonstrates interest in the welfare of ill and elderly.
  • Proof of current CPR and Hepatitis profile.
  • Aide Competency Evaluation Skills Checklist and written Skills Test.
