Job Summary

Cantex is seeking a skilled Home Health Aide to provide personal healthcare and related services to clients in their place of residence. The ideal candidate will work under the direction and supervision of a registered nurse, promoting patients' potential during their home health episode and collaborating with the clinical team and patient in preparation for discharge.

Responsibilities
  • Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care.
  • Assist patients with safe transfers and ambulation, adhering to agency policy.
  • Report changes and document pertinent information and care rendered to patients to ensure continuity of care.
  • Practice accepted infection control principles and provide a clean, safe, and comfortable environment.
  • Promote positive, respectful communication to patients, families, and other employees.
  • Contribute to the management and efficient operation of the agency, demonstrating effective time management skills.
  • Perform treatments and procedures according to agency policy.
  • Attend 12 hours of in-services per year to demonstrate commitment, professional growth, and competency.
  • Record clinical documentation in medical records and participate in weekly case conferences.
Requirements
  • High School Diploma preferred.
  • Current driver's license required.
  • At least one year of experience as a Home Health Aide or Nursing Assistant in a hospital, nursing home, or home health agency, or completion of a Certified Nursing Assistant or Certified Home Health Aide Program.
  • Good communication skills to relate professionally and positively to patients, caregivers, and clinical teams.
  • Ability to read, write, and speak the English language.
  • Demonstrates interest in the welfare of ill and elderly patients.
  • Proof of current CPR and Hepatitis profile.
  • Aide Competency Evaluation Skills Checklist and written Skills Test.
