Conducts comprehensive evaluations of physical, emotional, spiritual, and psychological well-being.
Develops, executes, and collaborates on the care plan alongside the interdisciplinary team.
Keeps the interdisciplinary team informed during patient care meetings.

Delivers direct nursing services to patients, ensuring the utmost quality of care under the guidance of the primary care physician.

Records patient data into the electronic health record system in compliance with established protocols.
Regularly updates the primary care physician as necessary and on a monthly basis.
Educates family members, hired caregivers, and nursing home staff on end-of-life care practices.
Conducts supervisory visits for LPNs and Hospice Aides routinely, adhering to policy guidelines.
Provides mentorship to new hires and students as required.

Fosters constructive and effective relationships with physicians, nurse practitioners, referral sources, nursing facilities, insurance case managers, patients, families, and colleagues.

Engages in on-call responsibilities, including evenings, nights, and/or weekends, to guarantee continuous patient care coverage.
Assists with staffing for in-home continuous care as necessary.
Aids in the admission process when required.
Procures durable medical equipment as needed to ensure patient safety and comfort.
Utilizes resources efficiently and responsibly (medical and office supplies, communication devices, computers).
Effectively manages time and schedules.
