Job Summary:
At Straub Medical Center, we are seeking a compassionate and skilled Patient Navigator, RN - Oncology to join our team. As a key member of our multidisciplinary team, you will play a vital role in providing exceptional patient care and navigation through the oncology treatment process.
Key Responsibilities:
• Collaborate with oncology team members to develop and implement patient care plans
• Provide education and support to patients and their families
• Facilitate communication between patients, families, and healthcare providers
• Coordinate care transitions and ensure seamless patient flow
Requirements:
• Bachelor's degree in nursing or related field
• Current Hawai'i Registered Nurse License
• Three years of clinical experience in oncology nursing
• Current BLS/HCP Card
