About the Role

Mental Health Association, Inc. is seeking a highly skilled and compassionate Bilingual Behavioral Health Case Manager to join our team. As a key member of our community outreach team, you will play a vital role in providing support and services to individuals with mental health and substance use challenges.

Key Responsibilities
  • Intake and Assessment: Conduct thorough intake meetings with clients, assess their needs, and develop individualized plans to support their recovery and well-being.
  • Case Management: Provide ongoing case management services, including coordination of medical and mental health services, housing support, and employment assistance.
  • Housing Stability: Assist clients in securing and maintaining safe and stable housing, including developing relationships with landlords and providing tenancy support.
  • Income and Benefit Maximization: Help clients access and maximize their benefits, including Social Security, EAEDC, and SNAP/Food Stamps.
  • Community Resources and Access: Connect clients with community resources, including medical insurance, ID, driver's license, and other essential services.
  • Participant Health and Wellbeing: Assist clients in coordinating medical and dental appointments, monitoring medications, and promoting healthy habits and physical activity.
Requirements
  • Education: Bachelor's degree in human services or a related field.
  • Experience: At least six months of experience working with individuals with mental health and/or substance use challenges or individuals with a history of housing instability.
  • Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in computer software and online systems.
