Clinical Care Professional Role Overview

Gulfside Healthcare Services, Inc. is seeking a skilled Clinical Care Professional to join our team. As a key member of our clinical staff, you will play a vital role in ensuring smooth operations and supporting the delivery of high-quality patient care. This position requires strong organizational skills, attention to detail, and excellent communication abilities.

Responsibilities:
  • Schedule home health visits and communicate relevant information to clients and clinicians as needed.
  • Take phone calls, direct them appropriately, and take verbal orders from referral sources when necessary.
  • Assist field staff with acquiring medical supplies and other needs.
  • Secure physician orders, chart paperwork, and enter data into the appropriate database program, fax or send orders to physicians for signatures within specified time frames as requested.
  • Problem-solve with customers, doctors, and field staff.
  • Fax lab results to physicians.
  • Assemble admission folders and ensure up-to-date information is included in the folders.
  • Support the Clinical Care Manager and Administrator as needed.
  • Maintain patient, staff, and agency confidentiality.
  • Field visits as needed, directed by the Administrator.

Requirements:
  • Graduate of an accredited school of nursing; registered as a Licensed Practical Nurse in the State of Florida required.
  • Knowledge of principles of nursing and rehab home health and their applications.
  • Servant attitude toward servicing clients and internal customers from all departments and business lines.
  • Time management skills, ability to meet deadlines, and work independently, exercising independent judgment and critical thinking.
  • Licensed driver with automobile in good working order and insured in accordance with Gulfside Healthcare Services requirements.
  • Computer literate with working knowledge of Windows/MS office.

Estimated Salary: $52,000 - $62,000 per year.
