About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Chapters Health System. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious illnesses.

Responsibilities
  • Provide comprehensive and efficient patient care visits in various clinical settings, working under protocol with physicians from our medical staff.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Consult with primary and attending physicians to ensure seamless integration of hospice care with primary or other care.
  • Participate in after-hours and weekend hospice house rounding and face-to-face coverage for hospice patients.
  • Document patient care and administrative activities thoroughly and accurately, ensuring proper CPT and ICD-10 coding.
Requirements
  • Master's degree in nursing and APRN certification recognized by CMS for Medicare billing.
  • Licensure by the state of Florida to work as an APRN and current DEA license.
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.
  • Knowledge and experience in medical practice and symptom management, as well as advanced clinical skills.
  • Strong interpersonal skills, communication, collaboration, and conflict resolution abilities.
