Home Hemodialysis Nurse

We are seeking a highly skilled and compassionate Home Therapy Registered Nurse to join our team. As a key member of our home dialysis program, you will play a vital role in providing exceptional patient care and support to individuals choosing home dialysis as their treatment modality.
Key Responsibilities- Provide patient training and ongoing support for home dialysis modalities, ensuring compliance with federal and state regulations.
- Conduct home visits to assess patient safety and environment, and evaluate patient performance and home environment as needed.
- Communicate effectively with patients, physicians, and team members to ensure seamless care coordination and continuity.
- Document all nursing services and interventions in the Electronic Medical Record, ensuring accurate and timely documentation.
- Participate in infection control monitoring and implementation, and be familiar with emergency equipment and procedures.
- Ensure clinic and patient supply inventory and usage are appropriate for patient prescriptions, and place orders in a timely manner.
- Continuously seek to improve patient and quality outcomes through tracking and trending of program and patient performance.
- Registered Nurse licensure and experience in home dialysis or related field.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Flexibility to work rotating shifts, including nights and weekends, as needed.
- Ability to travel to patient homes and clinics, as required.
