Job Title: RN Home Care Coordinator

Join our team as a RN Home Care Coordinator and play a vital role in delivering high-quality care to our patients. As a member of our interdisciplinary team, you will work closely with healthcare professionals to develop and implement personalized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Conduct thorough assessments of patients in their home environments to determine their individual needs and develop effective care plans.
  • Provide regular home visits to monitor patients' progress, address any concerns, and make adjustments to care plans as needed.
  • Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
  • Document patient information accurately and timely to ensure continuity of care.
  • Participate in family meetings and case conferences to ensure patient-centered care.
  • Develop and maintain relationships with patients, families, and healthcare providers to ensure high-quality care.

Requirements:

  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts.
  • Minimum of 2 years of experience working with a geriatric population.
  • Minimum of 2 years of experience working in a community health setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
