Job Title: Clinical Pharmacist

Join Advocate Aurora Health as a Clinical Pharmacist and play a vital role in delivering high-quality patient care. As a key member of our pharmacy team, you will work closely with healthcare professionals to develop and implement effective medication therapy plans.

About the Role:
  • Develop and maintain expertise in clinical practice, including appropriate drug therapy and prevention/resolution of drug-related problems.
  • Evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions.
  • Individualize drug therapy regimens based on patient size, organ functions, and disease states.
  • Facilitate medication-related continuity of care at admission, transfer, and discharge.
  • Review and accurately interpret medication orders.
  • Develop competency in dispensing and compounding medications consistent with good pharmaceutical practice.
  • Practice in compliance with department policies and procedures, as well as local, state, and federal regulations.
  • Ensure formulary adherence, recommend cost-effective therapies to providers, and minimize waste in daily practice.
  • Actively support procedures and initiatives to prevent medication events.
  • Report events through appropriate channels.
  • Support department medication safety and quality improvement activities.
  • Orient and train new pharmacy team members.
  • May work with interns, residents, and students to guide daily activities.
  • May be preceptor-in-training.
  • Provide direction to pharmacy technicians to ensure smooth workflow and service standards.
Requirements:
  • Pharmacist license issued by the state in which the team member practices.
  • Patient-facing teammates may be required to have immunization and non-vaccine injectable certifications.
  • Bachelor's Degree in Pharmacy.
  • No experience required.
