POSITION SUMMARY:
Under the guidance of the Chief Medical Officer and with clinical oversight from the Medical Director, the Geriatric Physician will provide medical care to participants. This includes assessing health needs and developing collaborative care plans with the interdisciplinary team. The physician will deliver direct patient care and evaluate the effectiveness of treatment plans. A collegial relationship with staff is essential to promote optimal, safe, and effective patient-centered care. Independent decision-making regarding medical needs and health regimens is expected, along with the execution of necessary medical procedures.

ESSENTIAL FUNCTIONS:
  • Conduct initial and semi-annual medical assessments for participants, developing and implementing care plans.
  • Assess the effectiveness of care plans and make necessary adjustments.
  • Gather comprehensive health histories and document findings systematically.
  • Perform physical examinations and order appropriate laboratory and diagnostic tests, documenting results effectively.
  • Conduct functional and mental status evaluations using standardized methods.
  • Manage changes in participant status and facilitate appropriate interventions.
  • Provide guidance and support to medical staff as needed.

TREATMENT RESPONSIBILITIES:
  • Deliver preventive health care and promote health among participants.
  • Manage acute and chronic illnesses in various settings, including health centers and participants' homes.
  • Adjust medications as necessary, ensuring cost-effectiveness.
  • Coordinate specialist consultations and oversee participants' interactions with medical specialists.
  • Educate participants and their families on health maintenance and chronic conditions.
  • Coordinate care during hospitalizations and communicate participant status to the interdisciplinary team.
  • Ensure availability for consultation and support 24/7 regarding patient care concerns.
  • Review patient assessments regularly with the team to address outstanding issues and new diagnoses.
  • Interpret diagnostic tests in accordance with current practice standards.
  • Perform in-office medical procedures as required.

ADDITIONAL RESPONSIBILITIES:
  • Provide in-service training as necessary.
  • Communicate program details to interested parties.
  • Collaborate with the interdisciplinary team to develop and monitor comprehensive care plans.
  • Maintain accurate documentation of healthcare services provided.
  • Participate in various organizational committees as requested.
  • Prepare and submit necessary reports on services rendered.
  • Attend relevant healthcare conferences to stay updated on current trends.
  • Ensure the privacy and confidentiality of sensitive information.
  • Advise leadership on medical and health-related matters.
  • Adhere to safety protocols when handling potentially infectious materials.
  • Perform other related duties as assigned.
