Job Summary

We are seeking a compassionate and skilled Hospice Licensed Practical Nurse (LPN) to join our team at FC Compassus LLC. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in their time of need.

About the Role

As a Hospice LPN, you will be responsible for providing direct, skilled nursing services to patients in accordance with their individualized Plan of Care. You will work collaboratively with our interdisciplinary team to ensure that patients receive comprehensive care that addresses their physical, emotional, and spiritual needs.

Responsibilities

• Provide skilled nursing services to patients in their homes, including wound care, medication management, and vital sign monitoring
• Develop and implement individualized care plans that address patients' unique needs and goals
• Collaborate with the RN Case Manager and other members of the care team to ensure seamless care coordination
• Educate patients and their families on self-care techniques and disease management
• Provide emotional support and counseling to patients and their families as needed

Requirements

• Current LPN licensure in the state of practice
• Experience in hospice care or a related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and empathetic approach to patient care
